Medicine Form
If your child requires medication to be taken during their time at school, you will need to complete our administration of medicine form and return it to us.
Please note that medication will only be administered by a member of school staff if it is absolutely necessary and this form has been completed by the child’s parent or guardian.

Application For Leave of Absence
If your child requires a leave of absence during term time, you will need to complete our Application for leave of absence and return it to us.
Please note that parents do not have any entitlement to take their child on holiday during term time and applications for a leave of absence will be considered only in exceptional circumstances.
